Financial
Our practice accepts cash, checks, major credit cards, and Care Credit. We will be more than happy to submit your insurance forms for you. Payment is required at the time services are provided. Our staff is happy to discuss payment options with you.
appointment Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. We understand that sometimes life happens without warning. A non-cancellation fee may be charged if multiple appointments are missed without cancellation.
